IAC: Tu-154 from Dagestan crashed down in 2010 in Domodedovo because of crew error

The reason of the crash of the Tu-154 aircraft belonging to the "Dagestani Airlines" at the Moscow "Domodedovo" Airport in December 2010 was in erroneous actions of the crew. This is the conclusion the Interstate Aviation Committee (IAC), which has published today the final report on the accident.

"The accident with the Tu-154M No. RA-85 744 happened because of the erroneous actions of the crew during the flight and landing with one engine running, which led to the approach of the aircraft to the runway in the position unfit for landing; its late landing onto the flying field; rolling out and collision with a ground elevation," says the statement posted on the website of the IAC.

The investigation has revealed that the development of the emergency began after an inadvertent engagement by the flight engineer of the fuel re-pumping during the climb, which "led to ripple of the instant fuel consumption," drop of engine speed and failure of the two outside engines. The tail propulsion engine No. 2 continued working; however, in spite of this, for more than two minutes the aircraft had serious problems with electric power, the "Interfax" reports.

Besides, the emergency could be caused by the crew's ignoring of the recommendations of the Flight Manual of the Tu-154M, by the absence on the part of the commander of an aircraft of due management and distribution of duties among the crew members and "by independent, not correct in all the cases, actions of the crew members, and lack of commander's training in crew resource management."

The IAC has also noted a complex wind situation by heights, which "had further contributed to the deviation of an aircraft from the landing course," the RIA "Novosti" reports.
